Job summary

Ariel Bay Rehabilitation & Care Center is seeking a compassionate Licensed Practical Nurse (LPN) to provide direct nursing care to residents under the supervision of the DON and RNs. You will administer medications, monitor residents, document care, and support the health care team.

The role involves supervising CNAs, adhering to infection control and safety standards, and participating in care planning to promote resident dignity and quality of life.

Qualifications

• Active New Jersey LPN license.
• Graduate of an accredited Practical Nursing program.

Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents under the supervision of the Director of Nursing and Registered Nurses. The LPN is responsible for administering medications and treatments, monitoring resident conditions, maintaining accurate documentation, and ensuring quality care is provided in accordance with facility policies and procedures.

Essential Duties and Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor residents for changes in condition and report findings to the appropriate healthcare team members.
• Perform nursing assessments and document resident care accurately and timely.
• Ensure physician orders are transcribed and followed appropriately.
• Supervise Certified Nursing Assistants (CNAs) and provide guidance as needed.
• Respond to resident needs, concerns, and emergencies promptly.
• Maintain compliance with infection control, safety, and regulatory requirements.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Promote resident dignity, rights, and quality of life.
• Participate in care planning and resident care conferences as assigned.

Qualifications

• Current and active New Jersey LPN License.
• Graduate of an accredited Practical Nursing Program.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.

Preferred Qualifications

• Long-Term Care, Skilled Nursing, Rehabilitation, or Subacute experience.
• Experience with Electronic Medical Records (EMR).
