Job summary

Avalon Rehabilitation and Healthcare Center in New Jersey is seeking an LPN to join our compassionate care team. Weekend Baylor availability from 7AM to 7PM is requested, and per diem opportunities are available on all shifts.

Qualifications

• Current, unencumbered LPN license in New Jersey.
• Valid BLS (CPR) certification is required.
• Ability to care for multiple residents in a fast-paced setting.
• Strong clinical judgment, communication, and attention to detail.

Responsibilities

• Provide patient care per standards and written plan of care.
• Follow care plans developed by Nurse Manager and team.
• Administer medications and treatments per plan and policies.
• Monitor residents, report changes promptly to supervisors.
• Maintain accurate and timely documentation.
• Support a safe, respectful, therapeutic environment.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of NJ
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
