Job summary

Seacrest Rehabilitation and Healthcare Center in New Jersey is seeking a Licensed Practical Nurse (LPN) for full-time, part-time or per-diem roles. Shifts include 7:00 AM–3:00 PM, 3:00 PM–11:00 PM, and 11:00 PM–7:00 AM with a $32–$39/hour pay range and a $3,000 sign-on for full-time staff.

We offer same-day pay, shift differentials, tuition reimbursement, health, vision and dental plans, and 401(k) with match.

Qualifications

• Current, unencumbered LPN license in New Jersey.
• BLS CPR certification required.
• Ability to care for multiple patients in a fast-paced setting.
• Strong clinical judgment and attention to detail.

Responsibilities

• Provide patient care per standards and plan of care.
• Follow individualized care plans from Nurse Manager and team.
• Administer medications per plan and policies.
• Monitor residents and report changes promptly.
• Maintain accurate documentation per orders and standards.
• Support a safe, respectful and therapeutic environment.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Jersey.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
