Job summary

Elara Caring is seeking a compassionate Licensed Practical Nurse (LPN) for full-time home health across York, Wells, and surrounding communities in southern Maine. You will provide skilled nursing care in patients' homes, follow care plans, monitor conditions, and educate patients and caregivers.

Qualifications

• Licensed Practical Nurse with an active Maine license.
• Graduate of an accredited practical nursing program.
• At least one year of acute care or comparable clinical experience.
• Current CPR certification (AHA or Red Cross).
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

We are looking for a compassionate Licensed Practical Nurse to join our growing southern Maine team. This full-time position serves patients throughout York, Wells, and nearby communities .

What You’ll Do

• Provide skilled nursing care in patients’ homes under the direction of an RN and physician
• Follow individualized plans of care
• Monitor patients and report changes in condition
• Administer medications and treatments within your scope of practice
• Educate patients and caregivers
• Complete timely and accurate clinical documentation
• Promote patient safety and independence
• Collaborate with nurses, therapists, physicians, and other members of the care team
• Participate in team meetings and the on-call rotation

What You’ll Need

• Current and unrestricted Licensed Practical Nurse license in Maine
• Graduate of an accredited practical nursing program
• At least one year of experience in an acute care or comparable clinical setting
• Current CPR certification through the American Heart Association or American Red Cross
• Strong communication, organization, and documentation skills
• Compassionate and patient-focused approach to care
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Ability to travel between patient homes throughout Portland and Cumberland County

The base salary for this position is $35.00 to $40.00 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
