Job summary

Elara Caring in Portland, ME is seeking a compassionate Licensed Practical Nurse to provide in-home skilled nursing services across Portland and Cumberland County. This full-time role supports patients in their homes under RN/physician guidance, delivering care with a focus on safety, comfort, and independence.

You will administer medications, educate families, document care, and collaborate with the care team.

Qualifications

• Current and unrestricted LPN license in Maine.
• Graduate of an accredited practical nursing program.
• At least one year of experience in an acute care or comparable clinical setting.
• Current CPR certification through AHA or Red Cross.
• Strong communication, organization, and documentation skills.

Responsibilities

• Provide skilled nursing care in patients’ homes under the direction of an RN and physician.
• Follow individualized plans of care.
• Monitor patients and report changes in condition.
• Administer medications and treatments within your scope of practice.
• Educate patients and caregivers.
• Complete timely and accurate clinical documentation.
• Promote patient safety and independence.

We are looking for a compassionate Licensed Practical Nurse to join our growing Portland team. This full-time position serves patients throughout Portland and Cumberland County.

What You’ll Need

• Current and unrestricted Licensed Practical Nurse license in Maine
• Graduate of an accredited practical nursing program
• At least one year of experience in an acute care or comparable clinical setting
• Current CPR certification through the American Heart Association or American Red Cross
• Strong communication, organization, and documentation skills
• Compassionate and patient-focused approach to care
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Ability to travel between patient homes throughout Portland and Cumberland County
