Job summary

Boost Home Healthcare - Fairfax West in Warrenton, VA is seeking an experienced Licensed Practical Nurse (LVN) to provide skilled nursing services in patients' homes under physician orders and RN supervision.

You will implement care plans, perform treatments, educate patients and families, and maintain accurate records while ensuring safety and quality of care in a home setting.

Qualifications

• Graduate of an approved school of practical nursing.
• Must be a Licensed Practical Nurse (LVN) in the state of posting.
• Minimum of one year as an LVN in home health care.
• Current BLS certification.

Responsibilities

• Provide nursing services, treatments and diagnostic and preventive procedures as assigned.

The qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. What you’ll be doing:

• Implement the nursing care plan for each patient.
• Provide nursing services, treatments and diagnostic and preventative procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Maintain accurate and complete records of observations, treatments, and care of patients.
