Job summary

Boost Home Healthcare is seeking a compassionate Licensed Practical Nurse to provide skilled nursing care in patients' homes, following physician orders and the plan of care under the supervision of a Registered Nurse.

The role includes implementing care plans, delivering nursing services and teaching patients and families to manage care at home, while maintaining complete records and promoting patient independence.

Qualifications

• Graduate of an approved school of practical (vocational) nursing.
• Licensed practical (vocational) nurse in the state of job posting.
• Minimum of one (1) year as an LVN in the Home Health Care Setting.
• Must have current BLS Certification.

Responsibilities

• Implement the nursing care plan for each patient.
• Provide nursing services, treatments and diagnostic and preventative procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Maintain accurate and complete records of observations, treatments, and care of patients.

Services are furnished in accordance with Boost Home Healthcare policy.
