Job summary

Boost Home Healthcare is seeking a Licensed Practical Nurse (LVN) to deliver skilled nursing services in patients' homes under physician orders and RN supervision. You’ll implement care plans, monitor conditions, and educate families to support recovery and independence.

Requirements include graduation from an approved LVN program, valid LVN license in the posting state, a minimum of one year in home health, and current BLS certification. Flexible scheduling is available.

Qualifications

Graduation from an approved LVN program and a valid LVN license in the posting state.
Minimum of one year as an LVN in the Home Health Care setting.
Current BLS certification is required.

Responsibilities

Implement the nursing care plan for each patient.
Provide nursing services, treatments and diagnostic and preventative procedures as assigned.
Initiate preventive and rehabilitative nursing procedures as appropriate.
Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
Teach and counsel the patient and family/significant others regarding nursing care needs and other related problems of the patient at home.
Maintain accurate and complete records of observations, treatments, and care of patients.

Education
