Job summary

Boost Home Healthcare is seeking a licensed practical nurse (LVN) to provide skilled home health care in patients' residences. The LVN will implement the care plan, perform nursing procedures, and monitor patient progress in coordination with the RN and physician.

Ideal candidates have an active LPN/LVN license in Florida, a minimum of one year of Home Health experience, and a current BLS certification. Join a team committed to respectful, patient-centered care and flexible scheduling.

Qualifications

• Graduate of an approved school of practical nursing.
• Licensed Practical Nurse (LPN/LVN) in the state posted.
• Minimum 1 year as an LVN in a Home Health Care setting.

Responsibilities

• Implement the nursing care plan for each patient in the home.
• Provide nursing services, treatments and preventative procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments and changes in condition.
• Teach and counsel patient and family regarding nursing care needs at home.
• Maintain accurate and complete patient records.

What you’ll be doing:

• Implement the nursing care plan for each patient.
