Job summary

Boost Home Healthcare in Warrenton, VA is seeking a licensed practical nurse to implement the care plan, administer nursing services at patients' homes, and coordinate with RNs to support recovery and wellness.

The ideal candidate has an active LVN license in Virginia, current BLS, and at least one year of home health experience. We offer flexible scheduling and ongoing training, with strong field support for the nursing team.

Qualifications

• Graduate of an approved school of practical nursing.
• Licensed Practical Nurse in the state of the posting.
• Minimum of one year as LVN in home health care.
• Current BLS certification.

Responsibilities

• Implement the nursing care plan for each patient.
• Provide nursing services, treatments and diagnostic procedures as assigned.

• Observe patient conditions and report changes to the physician and RN.

The qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. What you'll be doing:

• Implement the nursing care plan for each patient.
• Provide nursing services, treatments and diagnostic and preventative procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Maintain accurate and complete records of observations, treatments, and care of patients.
