Job summary

York Post Acute is seeking a Licensed Practical Nurse (LPN) to support residents with compassionate and high-quality care. The role covers day, evening, and night shifts with a comprehensive benefits mindset and opportunities for growth in a 80-bed sub-acute and long-term care setting.

Ideal candidates hold an active LPN license, valid BLS, and can work effectively within a multidisciplinary team to deliver person-centered care and maintain regulatory compliance.

Qualifications

• Active LPN license in the applicable state (Virginia noted) and valid BLS certification.
• Ability to manage multiple patients in a fast-paced healthcare setting.
• Strong clinical judgment and clear communication with patients, families, and staff.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow care plans developed by Nurse Manager and team.
• Administer medications and treatments in accordance with policies.
• Monitor residents, report changes promptly, and respond to needs.

SHIFTS: 7:00AM - 3:00PM, 3:00PM - 11:00PM, Or 11:00PM - 7:00AM

Hourly Range: $28.50 - $33.50/hr (Based on Experience & Incentives) ALL INCLUSIVE

No Frills Program for Full-Time Employees: If you opt for no benefits (no vacation, sick time, personal days, or holidays), you'll earn an additional $3/hour on top of your base pay.

Shift Differentials: $1.50 for 3pm 11pm & $1.00 For 11pm - 7am)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Virginia.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
