York Post Acute is seeking a Licensed Practical Nurse (LPN) to join our team in Virginia. We offer Full-Time, Part-Time, and Per-Diem opportunities with multiple shifts (7:00AM–3:00PM, 3:00PM–11:00PM, 11:00PM–7:00AM) and an all-inclusive hourly range of $28.50–$33.50 based on experience and incentives.

No-frills option adds $3/hour gross; shift differentials apply. At York Post Acute, you’ll provide compassionate nursing care, support residents’ needs, and collaborate with an interdisciplinary

Qualifications

• Current, unencumbered active LPN license in Virginia.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow care plans developed by Nurse Manager and team.
• Administer medications and treatments as prescribed.
• Monitor residents, report changes promptly, respond to clinical needs.
• Maintain accurate documentation per orders and policies.
• Support a safe, respectful environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Virginia.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
