Job summary

Sans Souci Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing team. The position involves direct patient care in the SNF and Long Term Care units, including medication administration (IVs when ordered), vital signs, and care planning.

The LPN will educate residents and families, coordinate with physicians and NPs, and supervise CNAs to ensure high-quality care in a supportive environment.

Qualifications

• Current LPN license is required.
• Experience with medication administration, including IVs, preferred.
• Ability to take and record vital signs and monitor patient outcomes.
• Experience educating residents and families and communicating with health care teams.
• Supervising CNAs and coordinating with physicians and NPs.

Licensed Practical Nurse (LPN) JOB DESCRIPTION:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
