Job summary

Buckeye PACE is seeking a Licensed Practical Nurse to deliver direct and indirect nursing care within participants' individualized plans. You will work closely with the Clinic Nurse Manager and care team to provide safe, accurate, and compassionate care, within established routines.

This role requires patient, dependable team members who value steady work, clear communication, and a calm, collaborative environment where participant wellbeing comes first.

Qualifications

• Graduate of an accredited nursing program with a current state LPN license.
• One year of experience with frail or elderly population or willingness to train including overnight travel.
• CPR and First Aid Certification or willingness to obtain after hire.

Responsibilities

• Provide direct and indirect nursing care to participants.
• Collect participant data and complete required documentation.
• Assist with planning, implementing, and evaluating individualized plans of care.
• Coordinate services by communicating participant needs to the care team.
• Participate in development of the IDT Plan of Care and ensure follow through.
• Provide treatments and care as prescribed.
• Educate participants and families on health needs and care instructions.
• Complete home visits when needed.
• Document all care in the EMR system.
• Support quality improvement activities.
• Follow established guidelines and safety protocols.
• Perform other duties as assigned.

Duties/Responsibilities

• Provide direct and indirect nursing care to participants
• Collect participant data and complete required documentation accurately and according to standards
• Assist with planning implementing and evaluating individualized plans of care
• Support coordination of services by communicating participant needs and changes to the care team
• Participate in the development of the IDT Plan of Care and ensure proper follow through
• Provide treatments procedures and care as prescribed and according to practice standards
• Educate participants and families on health needs and care instructions
• Complete home visits when needed
• Document all care in the electronic medical record system
• Support quality improvement activities
• Follow established guidelines policies and participant safety protocols
• Other duties as assigned

• Successful completion of background check physical drug screening TB Screening
