The Licensed Practical Nurse (LPN) at the CrisisCare Center is responsible for providing medical psychiatric assistance to adult consumers with mental illness or co-occurring disorders.
The LPN handles workflow processes, maintains confidentiality, demonstrates knowledge of co-occurring disorders and appropriate interventions, and works to minimize the disabling effects of mental illness and substance abuse disorders. The role involves empowering consumers to manage their health, prevent diseases, and improve overall wellness by implementing health strategies and incorporating disease management models addressing behavioral disorders.
CCBHC Model of Care
