Interim HealthCare of North Central Pennsylvania is seeking a Home Health Licensed Practical Nurse (LPN) in South Williamsport for overnight shifts. We offer one-on-one patient care, top pay of $25.00–$35.00 per hour, flexibility, online training, and growth opportunities.

Responsibilities include medication administration via g-tube, tracheostomy/ventilator care, vitals, assessments, and assisting with personal care while coordinating with the care team.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in PA.
• Six months experience in home care or healthcare setting required.
• CPR certification.
• Reliable transportation.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsabilités

• Medication administration and nutrition via g-tube
• Manage and monitor tracheostomy and ventilator care
• Work with a team to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist with full personal care tasks such as bathing, dressing, grooming, and oral hygiene
• Follow the individual’s plan of care
• Collaborate with interdisciplinary team to coordinate patient care
