Job summary

Interim HealthCare of North Central Pennsylvania in Millville is seeking a Home Health Licensed Practical Nurse (LPN) for overnight shifts. This role focuses on one-on-one patient care, medication administration and following individualized plans of care to support recovery at home.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in PA.
• Six months experience in home care or healthcare setting required.
• CPR certification.
• Reliable transportation.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Medication administration and nutrition via g-tube.
• Monitor for seizure activity and follow protocol.
• Urinary catheter changes.
• Work with a team to deliver quality patient care.
• Take patient vitals, do assessments, document progress and report changes to RN supervisor.
• Assist with full personal care tasks such as bathing, dressing, grooming, and oral hygiene.
• Follow the individual’s plan of care.
• Collaborate with interdisciplinary team to coordinate patient care.
