Interim HealthCare Inc. is hiring Home Health Licensed Practical Nurses (LPN) for shifts in Lock Haven. This role offers 1:1 patient care, flexible assignments, and strong pay between $25 and $35 per hour based on experience.

Qualifications

• Graduate of an accredited practical nursing program and active PA LPN license.
• Six months experience in home care or healthcare setting required.
• CPR certification highly desired.
• Reliable transportation is required for home visits.
• Excellent nursing skills, compassion and clear communication with patients.

Responsibilities

• Medication administration and nutrition via g-tube.
• Manage and monitor tracheostomy.
• Work with a team to deliver quality patient care.
• Take patient vitals, do assessments, document progress and report changes to RN supervisor.
• Assist with full personal care tasks such as bathing, dressing, grooming, and oral hygiene.
• Follow the individual’s plan of care.
• Collaborate with interdisciplinary team to coordinate patient care.
