Job summary

Healthwin Health & Rehabilitation in South Bend, IN is seeking compassionate LPNs to join our clinical team on 6A-6P shifts. Competitive pay and benefits offered.

You will administer medications, collaborate with RNs and CNAs, and support residents' well-being while following procedures and regulations.

Qualifications

• Current LPN license in IN.
• LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment

Responsibilities

• Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.
• Administer medications and treatments with precision and compassion
• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well-being
• Assist with resident comfort
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.
