Agape Care Group is seeking a Licensed Practical Nurse (LPN) to join our hospice team in Rock Hill, South Carolina. You will provide compassionate, routine care to patients under the supervision of a registered nurse and communicate closely with families and the care team.

In this role, you’ll participate in visits, coordinate with interdisciplinary staff, and help deliver comfort-focused care for patients facing life-limiting illnesses.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited school of nursing with a current state license as an LPN
• Experience: 2+ years of nursing in acute care; hospice experience preferred
• Reliable transportation; ability to sit, stand, bend, move intermittently and lift 80–100 lbs

Responsabilités

• Conduct patient visits and provide routine care under RN supervision.
• Communicate with care team, patient and family to coordinate care.
• Assist with hospice protocols and documentation as required.

• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.
