Job summary

ACG Hospice is seeking a Licensed Practical Nurse (LPN) to provide compassionate hospice care in patient homes or facilities. You’ll work under the supervision of a registered nurse, perform routine visits, administer medications, and communicate clearly with the care team, patients and families to ensure comfort and dignity.

Qualifications

• Graduate of an accredited nursing program with a current LPN license.
• 2+ years of nursing experience in an acute care setting; hospice experience preferred.
• Reliable transportation and ability to lift 80–100 lbs.

• Communicate with care team, patients, and families to ensure comfort.
• Assist with care planning and documentation in patient records.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
• Experience: 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.
