Job summary

Interim HealthCare Inc. of the Black Hills seeks a compassionate Home Health Licensed Practical Nurse (LPN) to join our team in Rapid City, SD.

You will provide skilled nursing visits in patients' homes under the supervision of an RN Case Manager, with 1:1 patient ratios and flexible assignments. Earn $24-$31 per hour, maintain independence, and advance your career through online training and CEUs, while benefiting from PTO, 401K, health insurance, and paid travel.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in South Dakota.
• Minimum of 1 year of LPN experience, ideally in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Compassionate, dependable, and committed to patient care.
• Organized and able to manage daily schedule independently.
• Strong communication with patients and families.
• Team player who values collaboration and learning.
• Comfortable using electronic medical records and completing timely documentation.
• Reliable transportation and current driver's license.

As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Providing skilled nursing visits in patients' homes under the direction of the RN Case Manager.
• Performing nursing procedures and treatments ordered by the physician and outlined in the plan of care.
• Monitoring patient conditions and reporting changes to the supervising RN.
• Reinforcing education provided by the RN to help patients and families manage their health with confidence.
• Administering medications and treatments within the LPN scope of practice.
• Accurately documenting all patient care and clinical observations.
• Collaborating with physicians, nurses, therapists, caregivers, and other members of the healthcare team.
• Helping patients maintain independence, dignity, and the highest possible quality of life.
