Interim HealthCare of the Black Hills is seeking a compassionate Home Health LPN in Rapid City, SD to provide skilled nursing visits under an RN Case Manager. You’ll educate, monitor, and support patients and families in their homes, promoting safety, independence, and high-quality care.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in South Dakota.
• CPR certification.
• Knowledge of state and federal home health regulations.

Responsibilities

• Providing skilled nursing visits in patients' homes under the direction of the RN Case Manager.
• Performing nursing procedures and treatments ordered by the physician and outlined in the plan of care.
• Monitoring patient conditions and reporting changes to the supervising RN.
• Reinforcing education provided by the RN to help patients and families manage their health with confidence.
• Administering medications and treatments within the LPN scope of practice.
• Accurately documenting all patient care and clinical observations.
• Collaborating with physicians, nurses, therapists, caregivers, and other members of the healthcare team.
• Helping patients maintain independence, dignity, and the highest possible quality of life.

As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Minimum of 1 year of LPN experience, ideally in home healthcare (preferred)
• CPR certification
• Knowledge of state and federal home health regulations

• Organized and able to manage their daily schedule independently.
• A strong communicator who enjoys building relationships with patients and families.
