Job summary

North End Rehabilitation & Healthcare Center is seeking an LPN for full-time, part-time, or PRN roles. The team emphasizes compassionate, high-quality nursing care and resident well-being in a 100-bed sub-acute and long-term care setting.

Responsibilities include medication administration, care plan implementation, and timely documentation, with shift options including 7p-7a and daily pay. Massachusetts licensed LPN with BLS is required.

Qualifications

• LPN license in MA is required.
• Valid BLS/ CPR certification required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills needed.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans with nurse manager and team.
• Administer medications per policy and POC.
• Monitor residents and report changes promptly.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Massachusetts.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
