Job summary

Cliveden Rehabilitation and Nursing Center in the Mt Airy section of Philadelphia is seeking a compassionate Licensed Practical Nurse (LPN) to join our team on a part-time basis. The LPN provides direct resident care, administers medications, and coordinates with direct care staff to ensure quality service.

CPR certification and PA LPN licensure are required; one year of long-term care is a plus. We offer a benefits package including health, dental and vision insurance as well as disability

Qualifications

• Graduate of an accredited nursing program and LPN license required.
• Active CPR certification required; Pennsylvania license or multi-state license.
• One year direct-care experience in long-term care preferred but not required.

Responsibilities

• Provide direct nursing care to residents per shift and plan of care.
• Administer treatments and medications and supervise direct care staff as needed.
• Ensure Flow of Care is followed and coordinate with the interdisciplinary team.

Education

LPN Rate: $31 to $36.50 per hour.

Job Description

As a member of the interdisciplinary team, the Licensed Practical Nurse (LPN) assumes responsibility and accountability for nursing services delivered to assigned residents of a designated unit for one shift. The LPN Nurse provides direct care, administers treatments and medications, organizes and distributes daily assignments to direct care staff consistent with staff competency and each individual resident's comprehensive resident assessment and plan of care. Supervises direct care staff and makes decisions about resident care needs during shifts concerning scope of clinical competence, consistent with facility policies and procedures. Ensures that “Flow of Care” is followed.

Education/Training/Work Experience

• Education: Graduate of accredited school of nursing
• Certificate/Licenses: Current, active license as Licensed Practical Nurse in Pennsylvania or current Multi-State license. Active CPR Certification required
• Work Experience: One year experience providing direct care to residents in long term care setting preferred but not required. Prior experience as Charge Nurse or similar supervisory experience preferred but not required.
