Cliveden Rehabilitation and Nursing Center in the Mt Airy section of Philadelphia is hiring caring LPNs for a part‑time role. Join our team and provide high‑quality care to residents in a supportive environment.

The LPN administers treatments and medications, coordinates with staff, and ensures care plans are followed. PA LPN license and CPR required; prior long-term care experience preferred but not required.

Qualifications

• Graduate of accredited school of nursing.
• Current, active PA LPN license or Multi-State license; CPR certification required.
• One year of direct resident care in long-term care preferred but not required; prior supervisory experience preferred but not required.

Responsibilities

• Provide direct care, administer treatments and medications as prescribed.
• Organize and distribute daily assignments to direct care staff consistent with resident plans.
• Supervise direct care staff and make shift decisions about resident care within policy.
• Ensure Flow of Care is followed and care standards are met.

Job Description

As a member of the interdisciplinary team, the Licensed Practical Nurse (LPN) assumes responsibility and accountability for nursing services delivered to assigned residents of a designated unit for one shift. The LPN Nurse provides direct care, administers treatments and medications, organizes and distributes daily assignments to direct care staff consistent with staff competency and each individual resident's comprehensive resident assessment and plan of care. Supervises direct care staff and makes decisions about resident care needs during shifts concerning scope of clinical competence, consistent with facility policies and procedures. Ensures that “Flow of Care” is followed.

Education/Training/Work Experience:

• Education: Graduate of accredited school of nursing
• Certificate/Licenses: Current, active license as Licensed Practical Nurse in Pennsylvania or current Multi-State license. Active CPR Certification required
• Work Experience: One year experience providing direct care to residents in long term care setting preferred but not required. Prior experience as Charge Nurse or similar supervisory experience preferred but not required.
