Job summary

OnShift, Inc. is seeking a Licensed Practical Nurse (LPN) to join our team at Woodlands. You will perform medication administration, assess residents, and update care plans to ensure high-quality care in a supportive environment.

The ideal candidate holds an Oklahoma LPN license with at least one year in a skilled nursing or long-term care setting, and is committed to the well-being of residents and teamwork.

Qualifications

• Current Oklahoma LPN license in good standing.
• Minimum 1 year of experience in Skilled Nursing or Long-Term Care.

Responsibilities

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure residents’ chart is current
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN)License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
