Job summary

The Wilshire Skilled Nursing & Therapy is seeking a dedicated Licensed Practical Nurse (LPN) to join our Oklahoma City team. This full-time role covers the 2pm-10pm shift and focuses on delivering high-quality patient care in a supportive environment.

Responsibilities include medication administration, patient assessment, care plan updates, and wound care. Oklahoma LPN license with 1+ year of SNF/LTC experience required; benefits and career development opportunities are provided.

Qualifications

• Current and unencumbered Oklahoma LPN License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.

Responsibilities

• Obtain and administer prescribed medications and treatments per nursing techniques.
• Observe residents and report significant changes to chart and care team.
• Take vital signs and assess changes in condition.
• Keep family and physician informed of health changes.
• Modify care plans and ADL flow sheets as conditions evolve.
• Provide wound care and prepare injections or enemas.
• Collect samples and monitor catheters.

Hiring Full 2pm-10pm

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure residents’ chart is current
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN)License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
