Claremore Skilled Nursing & Therapy is seeking a licensed Practical Nurse (LPN) to join our supportive, well-structured team in Oklahoma. This full-time role offers 12-hour rotating shifts and a competitive benefits package.

Responsibilities

• Obtain and administer prescribed medications and treatments.

• Communicate changes to residents’ families and physicians.
• Modify care plans in response to condition changes.
• Provide wound care and perform injections/ enemas as needed.

Available Shift (With Shift Differentials):

6am-6am (You will have 3-4 days off per week with 12-hour rotating shifts.

• Week 1: Mon, Tue, Sat*, Sun*
• Week 2: Wed, Thu, Fri

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure residents’ chart is current
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN)License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
