Job summary

Reliant Care Management Company is seeking LPNs for day or night shifts (7-7) with relocation assistance and a $10,000 bonus. Responsibilities include assessments, admissions, discharges, wound care, and leading CNAs/CMTs in a patient-centered environment. Rates are negotiable to attract qualified candidates.

Applicants must hold an LPN license and demonstrate empathy, integrity, and competence. Telecommuting is not available; this is an in-house clinical care position with comprehensive

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
