Reliant Care Management Company is seeking an LPN to join our care team in Missouri. The role focuses on assessing residents, administering care, and ensuring accurate documentation and communication with physicians and guardians.

You will lead CNAs and CMTs, manage admissions and discharges, and provide wound care following facility protocols. A compassionate, ethical, and competent approach is essential for success.

Qualifications

• LPN licensure required.
• Care delivery experience preferred.
• Ability to follow strict protocols and charting requirements.

Responsibilities

• Conduct assessments, follow protocols, report changes, and document findings.
• Complete admission duties: MAR, TAR, POS, chart setup, physician orders, and meds.
• Manage discharges: document, notify physician/guardian, arrange transport.
• Provide wound care per protocol: irrigate, pack, dressings.
• Lead CNAs and CMTs while ensuring facility policies are followed.
• Respond effectively to emergencies.

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
