The LPN will perform assessments, admissions, discharges, wound care, and lead CNAs while ensuring safety and adherence to facility protocols, particularly when interacting with behavioral health residents.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations

• Will be working with Behavioral Health Residents
