Job summary

SCO Family of Services in New York seeks an LPN to lead community health activities for children in our care. You will complete health and physical development assessments, track progress, and promote positive health behaviors.

You will connect families with medical and dental services, support immunizations, and chair the health advisory committee while responding to medical emergencies and urgent needs.

Qualifications

• Licensed Practical Nurse with training in public health and health administration.
• Experience in health education and maternal/child health.
• Ability to coordinate with medical services and families.

Responsibilities

• Complete health and physical development assessments of all children enrolled in services.
• Maintain a comprehensive health services program that tracks each child’s progress and promotes positive health behaviors.
• Support parents in creating a medical home that involves current immunizations, annual physical and dental examinations, and ongoing primary and preventive care.
• Provide linkages to medical and dental services as well as assistance in obtaining health insurance.
• Screen children’s health and eyesight when necessary.
• Chair the health advisory committee.
• Provide support to the center staff regarding medical emergencies and screening children’s immediate medical needs.
