Job summary

Ascend Health’s Ascend Hospice is hiring a Licensed Practical Nurse (LPN) for the Central Team in New Jersey. The LPN will provide compassionate clinical care, observe patient needs, and collaborate with the Hospice Nurse to manage symptoms and plan care.

The role requires strong communication, teamwork, and the ability to document thoroughly. Licensing in the state is required, with preference for hospice or long-term care experience and opportunities for growth within a supportive organization.

Qualifications

• Hold a valid and current LPN license in the state of operation.
• Minimum 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels.
• Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and all other meetings as assigned.
• Maintains sufficient and appropriate home care supplies for adequate patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides as assigned
