Job summary

Ascend Hospice & Palliative Care NJ is hiring a Licensed Practical Nurse to provide compassionate, patient-centered care in a home-based hospice setting. The LPN will observe, document, and respond to the physical, emotional, and nursing needs of patients and families.

Under supervision of the Hospice Nurse and IDC team, you will administer prescribed care, support symptom management, and collaborate with an interdisciplinary team to ensure consistent, high-quality care.

Qualifications

  • Valid and current LPN license in NJ.
  • Minimum 1 year of clinical experience.
  • Long Term Care/Home Care/Hospice experience preferred.
  • Strong verbal and written communication skills.
  • Ability to work within an interdisciplinary team.

Responsibilities

  • Observe and document patient and family needs on an ongoing basis.
  • Consult with Hospice Nurse on patient status and symptom management.
  • Deliver care as prescribed by physician under supervision.
  • Monitor and document patient/family input and response.
  • Maintain continuity of patient care through communication with families and other health professionals.
  • Participate in Interdisciplinary Team Meetings and other assigned meetings.
  • Maintain home care supplies for patient care management.
  • Collect documentation and paperwork as requested.
  • Assist with clinical billing as needed.
  • Participate in orientation of new hospice aides.

Now Hiring -Licensed Practical Nurse

TheLicensed Practical Nursewill be responsible for, but are not limited to:

  • Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis
  • Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels
  • Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team
  • Monitors and documents patient/family input and response to care
  • Maintains continuity of patient care through ongoing communication with families and other health care professionals
  • Participates in Interdisciplinary Team Meetings and all other meetings as assigned
  • Maintains sufficient and appropriate home care supplies for adequate patient care management
  • Collection of documentation and paperwork as requested by the Hospice Nurse.
  • Assistance with clinical billing as deemed necessary
  • Participates in orientation of new hospice aides as assigned

Education:

Associate's Degree in a related field

Professional Experience:
  • Hold a Valid and current LPN license within the state of operations
  • 1 year of clinical experience
  • Long Term Care/Home Care/Hospice experience preferred
