Job summary

Ascend Hospice in Lee's Summit, MO seeks a licensed practical nurse to join our compassionate care team. The role focuses on delivering patient- and family-centered care under the supervision of the Hospice Nurse and IDC team.

You will collaborate with an interdisciplinary team, monitor patient needs, and participate in family education and care planning. A state LPN license and 1 year of clinical hospice experience are preferred.

Qualifications

• Hold a valid and current LPN license within the state of operations.
• 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels.
• Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and all other meetings as assigned.
• Maintains sufficient and appropriate home care supplies for adequate patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides as assigned

Hours: M-F 8:30AM- 5PM
