Job summary

Ascend Hospice & Palliative Care MO is hiring a Licensed Practical Nurse to support patients in the Liberty, MO area. The role focuses on direct patient care under supervision of the Hospice Nurse and IDC team.

Responsibilities include symptom management, care coordination with the interdisciplinary team, and documentation. Associates degree required; Missouri LPN license required; hospice/home care experience preferred. Monday to Friday hours 8:30 a.m. to 5 p.m. on-site position.

Qualifications

• Hold a valid LPN license in the state of operations.
• 1 year of clinical experience.
• Hospice/Home Care experience preferred.
• Strong verbal and written communication skills.
• Able to work as part of an interdisciplinary team.

Responsibilities

• Observe and document the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consult with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels.
• Deliver physical and psychological care as prescribed by the attending physician, supervised by the Hospice Nurse and approved by the IDC team.
• Monitor and document patient/family input and response to care.
• Maintain continuity of patient care through ongoing communication with families and other health care professionals.
• Participate in Interdisciplinary Team Meetings and all other meetings as assigned.
• Maintain sufficient and appropriate home care supplies for adequate patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides as assigned.
