Job summary

Ascend Hospice & Palliative Care KS is seeking a Weekend Licensed Practical Nurse to support patient care Fri–Sun with 8:00 AM to 8:30 PM shifts. Duties include observation, symptom management, and collaboration with the Hospice Nurse and IDC team.

LPN license in KS required and hospital/home care experience preferred. The role requires effective communication, interdisciplinary teamwork, and commitment to compassionate end‑of‑life care in patients' homes.

Qualifications

• Hold a Valid and current LPN license within the state of operations.
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis
• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels
• Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team
• Monitors and documents patient/family input and response to care
• Maintains continuity of patient care through ongoing communication with families and other health care professionals
• Participates in Interdisciplinary Team Meetings and all other meetings as assigned
• Maintains sufficient and appropriate home care supplies for adequate patient care management
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary
• Participates in orientation of new hospice aides as assigned

Education:

Associate's Degree in a related field

Professional Experience:
