Summary

Provides basic nursing care by assisting in the assessment of the resident on admission and on an ongoing basis. Remains knowledgeable about and communicates to the appropriate interdisciplinary team (IDT) member any significant change in condition. Accurately documents all assessments. Seeks guidance if unsure of any assessment findings. Participates in the care planning process for the resident. Updates the care plan when specific interventions indicate the need for revision. Communicates the plan to the appropriate IDT members along with the residents and their families. Implements necessary and appropriate nursing interventions relevant to the plan of care, including nursing orders, physician orders, and residents'/families' educational and environmental needs. Ensures interventions such as medication administration and treatments are accomplished in a safe and timely manner. Administers IV fluids only if IV certified. Documents interventions accurately and timely. Uses critical thinking skills when dealing with residents' changes in condition. Notifies the physician timely using the SBARR communication tool. Maintains composure during crises such as family dissatisfaction, staff disputes, or inclement weather, in a calm and professional manner. Monitors residents' care closely to ensure effective delivery of the care plan. Identifies problem areas and uses the QAPI process for immediate resolution. Remains aware of residents' status to quickly identify behavioral changes or other signs and symptoms. Responds to call lights and assists with residents' ADLs as needed. Completes at least twelve hours of nursing-related continuing education, staying current with best practices through Bethesda's policies and self-study. May be assigned other duties as appropriate.

Job Qualifications
  • Education from an accredited school of nursing resulting in an LPN license
  • Current LPN license in the state of employment
  • CPR and AED certification required or willing to obtain within six months
  • IV certification required or willing to obtain within six months
  • Long-term care experience preferred
  • Ability to assess basic nursing care needs of geriatric residents
  • Excellent communication skills to interact with residents, visitors, and staff
  • Strong time management and organizational skills
  • Ability to stand and walk for long periods
  • Ability to lift heavy objects using proper techniques and assistance when necessary
  • Capacity to transfer and move patients weighing over 200 lbs with assistance and proper techniques
  • Ability to push and pull equipment over 40 lbs using proper techniques and assistance
  • Manual dexterity, coordination, and skillful use of hands for working with patients and equipment
  • Visual acuity, including color discrimination
  • Ability to perform documentation electronically and in writing
