Summary

Provides basic nursing care by assisting in the assessment of residents on admission and ongoing. Communicates significant changes to the interdisciplinary team (IDT), documents assessments, participates in care planning, and implements nursing interventions. Ensures safe medication administration and treatments, administers IV fluids if certified, and documents interventions accurately. Uses critical thinking for changes in residents' conditions and communicates with physicians using SBARR. Maintains composure in crises, monitors care effectiveness, and addresses problem areas using QAPI. Answers call lights, assists with ADLs, and completes continuing education. May perform other duties as assigned.

Job Qualifications

• Graduation from an accredited nursing program leading to LPN licensure
• Current LPN license in the state of employment
• CPR and AED certification required or willing to obtain within six months
• IV certification required or willing to obtain within six months
• Long-term care experience preferred
• Ability to assess geriatric nursing care needs
• Excellent communication skills
• Time management and organizational skills
• Ability to stand, walk, lift, and transfer patients safely
• Manual dexterity and visual acuity
• Ability to perform documentation electronically and in writing
