Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in patients' homes.

• Provides services within the scope of practice defined by state laws, in accordance with the care plan, and in coordination with the healthcare team. May not alter the care plan.
• Instructs patients' families on care and maintaining a healthy environment for the patient.
• Observes, records, and reports to the supervising nurse or physician the patient's response to treatment and any changes in condition daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice
• Current Driver's License, vehicle insurance, and access to reliable transportation
• CPR Certification required
