Job summary

Greenbrier Senior Living is seeking a compassionate LPN for The Rivers at Maryland Farms in Brentwood, TN. This full-time role includes day and night shifts (7 AM–7 PM, 7 PM–7 AM) with PRN options. Earn a $2,500 sign-on bonus and join a supportive leadership team dedicated to excellent resident care and teamwork.

As an LPN, you will follow individualized care plans, administer medications, perform assessments, and coordinate with the care team to ensure safety and comfort for residents.

Qualifications

• Active Tennessee LPN license required
• Current CPR certification
• High school diploma or GED
• One year of related nursing experience preferred
• Senior living, Assisted Living, or Memory Care experience is a plus
• Ability to exercise sound clinical judgment and respond appropriately to resident needs
• Ability to meet the physical requirements including assisting with transfers and mobility

Responsibilities

• Provide direct resident care per individualized plans of care.
• Administer medications per physician orders and policies.
• Perform resident assessments per state regulations and community policies.
• Monitor residents for changes in condition and communicate to leadership.
• Document resident observations and care provided during shifts.
• Maintain and update individualized resident care plans.

• Provide guidance to other associates as appropriate.

What You’ll Do

As an LPN at The Rivers at Maryland Farms, you’ll play an important role in providing exceptional care and creating a safe, comfortable environment for our residents.

• Provide direct resident care while following individualized plans of care.
• Administer medications in accordance with physician orders, state regulations, standards of practice, and community policies.
• Perform resident assessments as required by state regulations and community policies.
• Monitor residents for changes in condition and promptly communicate concerns to appropriate leadership.
• Document resident observations, clinical procedures, communications, and care provided during your shift.
• Maintain and update individualized resident care plans.
• Collaborate with our nursing, caregiving, and leadership teams to ensure residents receive excellent care.
• Communicate effectively with residents, families, physicians, and other members of the care team.
• Provide guidance and direction to other associates as appropriate.
• Help create a positive, supportive, and team-focused environment for residents and associates.

What We’re Looking For

• Active Licensed Practical Nurse (LPN) license in Tennessee
• Current CPR certification
• High school diploma or GED

• Strong communication and teamwork skills
