Job summary

You will provide high-quality nursing care, administer medications, monitor residents, document care, and work with CNAs, RNs and physicians to support safety and quality of life.

Qualifications

• Certified nursing skills appropriate for LPNs.
• Strong communication with residents, families, and the team.
• Ability to manage multiple residents with accuracy and empathy.
• Proven organizational abilities in a clinical setting.
• Experience in skilled nursing is a plus.

Responsibilities

• Provide high-quality nursing care to residents in accordance with individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents’ conditions and report changes to the RN and medical team.
• Complete accurate and timely clinical documentation.
• Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.
• Assist with assessments, care planning, and resident needs.
• Promote resident safety, dignity, comfort, and quality of life.
• Communicate effectively with residents and their families.
