Job summary

DocGo is seeking a Licensed Practical Nurse (LPN) for part-time home health visits in Louisville, KY. You will perform health assessments, document histories, administer vaccines and medications, and conduct tests in patients' homes.

Requirements include an active LPN license, 2+ years of LPN experience, BLS, and a valid KY driver’s license. Weekend/weekday availability is needed; travel and patient interaction are core aspects of the role.

Qualifications

• LPN with current license and eligibility to practice in Kentucky.
• Minimum 2 years of LPN experience.
• Phlebotomy/Blood withdrawal experience.
• Pediatrics experience preferred.
• BLS certification required.
• Kentucky Driver's License with clean driving record.
• Strong communication skills and ability to work independently.

Responsibilities

• Drive to patients' homes for health assessments and visits.
• Document medical histories and vital signs accurately.
• Administer vaccines and medications per protocol.
• Perform venipuncture and other required tests.
• Complete necessary documentation and maintain HIPAA confidentiality.

Position Overview

You will be providing health assessments and home health visits for our patients. We are seeking qualified Licensed Practical Nurses (LPN).

Job Duties:

• Driving to patient's homes and conducting home health visits independently
• Properly obtain and document medical histories
• Obtain consent, check patient history, and administer immunizations
• Complete documentation of assessment and/or interventions of patient
• Responsible for home health visits
• Minor troubleshooting of test systems and takes appropriate action according to predetermined protocols
• Identifies problems which may adversely affect test performance or reporting of test results and take appropriate action including supervisor notification
• Performs all relevant diagnostic tests as required
• Performs venipuncture as required
• Complete accepted medical forms
• Conduct weekly equipment check and report any equipment malfunctions
• Administer vaccines
• Administer medication to patients and measure vital signs.
• Monitor the patient's progress and changes in behavior.
• Recognize and treat any medical condition with appropriate techniques, equipment, and knowledge of all State and County protocols
• Recognizes situations which require the immediate attention of a physician or emergency care
• Interact with patients and their families in a compassionate manner
• Interact with clinicians, other health professionals and staff in a courteous and professional manner
• Effectively communicate and interact with patients and or agencies
• Participate in ongoing professional development activities
• Maintain a positive and professional work environment
• Maintain the confidentiality of all Personal Health Information as dictated by HIPAA laws
• Participate in community events organized through DocGo Partnerships.
• Perform Health Screenings and other requested services at scheduled community events.
• Other duties as assigned

Requirements:

• LPN License
• Blood Withdrawal / Phlebotomy experience
• Work experience with pediatrics
• A minimum of 2 years of LPN experience
• BLS with American Heart Association
• Valid Kentucky Driver's License (minimum of 2 years) and acceptable driving record
• Excellent verbal communication skills
• Ability to lift 50 - 60 lbs
• Patient bedside experience preferred (preferred but not required)
• Spanish speaking (preferred but not required)
• Must be comfortable driving and making home health visits independently
