Job summary

Aviata Health Group is seeking a dedicated LPN caregiver to provide direct patient care in a skilled nursing center in Jacksonville, FL. You will support residents under the supervision of a licensed nurse and contribute to care plans.

You will supervise CNAs, participate in falls and pressure sore programs, document evaluations, and perform additional nursing duties as assigned to ensure quality resident care.

Qualifications

• Must possess a current, active LPN license in the state.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Candidate must be compassionate and dedicated to high-quality care.

Major Responsibilities

The Licensed Practical Nurse (LPN) caregiver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.
• Other duties as assigned.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
