STAT Home Health is hiring a full-time Licensed Practical Nurse (LPN) in Hammond, Louisiana. Join a dedicated home health team and provide nursing care under the direction of an RN, coordinating with physicians and other care providers to ensure optimal patient outcomes.

Formación

• Licensed Practical Nurse (LPN) license in Louisiana is required.
• 1 year minimum clinical experience as an LPN.
• Current CPR Certification.

Job Duties:

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN

Qualifications:

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum 1 year clinical experience as an LPN
• Current CPR Certification
• Compassionate and great time management skills
• Reliable transportation to conduct patient visits at their place of residence or facility
