STAT Home Health is seeking Licensed Practical Nurses (LPNs) in Shreveport, LA to provide skilled nursing care at patients' homes or facilities under supervision of an RN.

Formación

• Current Louisiana LPN license required.

Responsabilidades

• Provide nursing and supportive care under the direction of the RN.
• Coordinate with physicians and home health team to ensure optimal patient care.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition to the RN.

Qualifications:

• Current Louisiana Licensed Practical Nurse (LPN) license required

Job duties:

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
