Job summary

St. Joseph Hospice in Baton Rouge, LA is seeking a Full-Time Licensed Practical Nurse (LPN) to join our compassionate hospice team. You will provide nursing and supportive care under the direction of the RN, ensuring comfort and quality of life for patients.

Qualifications include a Louisiana LPN license, 2–3 years of LPN experience, current CPR, and reliable transportation. Hospice experience is preferred.

Qualifications

• Current Louisiana LPN license required.
• Minimum 2-3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care under the direction of the RN.
• Communicates with attending physicians and hospice team to coordinate care.
• Documents all services on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid hospice benefits.
• Reports changes in patient condition to RN.

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
