Job summary

Willow Grove Post Acute is hiring a Licensed Practical Nurse (LPN) to join our compassionate care team at Willow Grove, PA. We offer full-time, part-time, and PRN opportunities with daily pay and starting wages from $30–$35 per hour inclusive.

You will provide patient care per care plans, administer medications, document accurately, and collaborate with the interdisciplinary team to support residents' well-being in a sub-acute and long-term care setting.

Qualifications

• Active LPN license in Pennsylvania.
• BLS CPR certification required.
• Ability to manage multiple patients in a fast-paced environment.
• Strong clinical judgment and attention to detail.

Responsibilities

• Provide patient care per standards and plan of care.
• Follow care plans from Nurse Manager and interdisciplinary team.
• Administer medications as prescribed and document accordingly.
• Monitor residents, report changes promptly, respond to needs.
• Maintain accurate documentation compliant with orders and regulations.
• Support a safe, respectful environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician's plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents' conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Pennsylvania.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
