St. Joseph Hospice in Baton Rouge, LA is hiring a Full-Time Licensed Practical Nurse (LPN) to join our patient-focused team. You will provide nursing and supportive care under the direction of the RN, coordinate with physicians and other hospice staff, and document services per plan of care.

Qualifications

• Valid Louisiana LPN license is required.
• 2–3 years of LPN experience.
• CPR certification required.

• Hospice experience preferred.

Responsabilités

• Provides nursing and supportive care under the direction of the RN.
• Coordinates with attending physicians, hospice team and other agencies to ensure optimal patient care.
• Documents services provided and submits forms per Plan of Care.
• Demonstrates knowledge of Medicare/Medicaid hospice regulations.
• Reports changes in patient condition and needs to the RN.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN)
• Current CPR certification required
• Have dependable transportation, driver’s license, and car insurance
• Previous Hospice experience preferred

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
